Health Management: Functions of Management


Management is nothing but about the process of getting things done through others. Various experts have classified the functions of management as follows:

As per George & Jerry, “Fundamental functions of management are four i.e. planning, organizing, actuating and controlling”.

As per Henry Fayol, the father of principles of management has classified managerial functions as follows: Planning, including forecasting, Organising, Commanding, Coordinating, and  Controlling.

As per Koontz & O'Donell five i.e. Planning, Organizing, Staffing, Directing & Controlling.

As per Luther Gullick and Urwiik POSDCORBE’ where P stands for Planning, O for Organizing, S for Staffing, D for Directing, Co for Co-ordination, R for reporting, B for Budgeting & E for evaluation.

Later on, Mr. Peter Drucker, the father of modern management has added: "innovation" & "managing change" as another managerial functioning on the above list. 

Planning: In simple words, it is deciding in advance - what to do, when to do & how to do? It bridges the gap between where we are & where we want to be. It is essential to decide how to go about achieving a certain task, breaking the job into several component activities, deciding the sequence in which the tasks will be done, entrusting the person who will do the task & when, etc.

Organizing: As per Fayol, “To organize a business is to provide it with everything useful to its functioning i.e raw materials, tools, capital/ finance, and personnel.” Thus, it involves bringing together the manpower and material resources for the achievement, of objectives laid down by the organization.

Staffing: According to Kootz & O’Donell, “Managerial function of staffing involves manning the organization structure through the proper and effective selection, appraisal & development of personnel to fill the roles designed un the structure”.  The main purpose of staffing is to put the right man on the right job i.e. square pegs in square holes and round pegs in round holes. The responsibility of filling up the positions lies with the senior management. Staffing involves:

  1. Manpower planning (estimating manpower in terms of searching, choose the person, and giving the right place).
  2. Recruitment, Selection & Placement.
  3. Training and development
  4. Remuneration
  5. Performance appraisal 
  6. Promotions & Transfer.

Directing: Direction is nothing but an inert-personnel aspect of management which deals directly with influencing, guiding, supervising, motivating sub-ordinate for the achievement of the goals of the organization. A manager directs the staff to work & guides how tasks will be performed. He also keeps a watch on the performance & ensures the appropriate quantity and quality of work. It has the following elements:

  1. Supervision
  2. Motivation
  3. Leadership
  4. Communication

Coordinating: It is a conscious and rational process of putting together various departments/ people of an organization and uniting them into a team to achieve goals in an effective manner. It is essential to ensure all members of the team are working in an integrated manner & tasks are being done in a coordinated and mutually supportive way.

Reporting/ Reviewing: Periodic review & reporting of a situation should be done to know if tasks are being done according to the plan or not & if some corrective measures have to be undertaken.

Budgeting: Financial provisions should be made and expenditure should be monitored so as to remain within the grants that are sanctioned.

Evaluation: It helps to determine how well the works have been done and what progress is being made towards achieving the goals. It is periodically assessed to make necessary changes at the top planning, policy, strategy, etc level.

According to modern management, there are four broad managerial functions:

  1. Planning
  2. Organizing
  3. Leading
  4. Controlling

कब्ज से बचाव के 5 उपाय

कब्ज एक बहुत ही आम समस्या है| यहां कब्ज को रोकने के लिए पांच सुझाव दिए गए हैं|

1. गतिशीलता: एक गतिही एवं शिथिल जीवन शैली से बचें| दिन में पर्याप्त व्यायाम करें -विशेष रूप से चलना (कम से कम 30 मिनट से 1 घंटे तक) महत्वपूर्ण है| 

2. पर्याप्त हाइड्रेशन: बहुत सारे तरल पेय पदार्थ लें (प्रतिदिन कम से कम 2.5 से 3 लीटर), विशेष रूप से पानी और फलों के रस| कॉफी और सॉफ्ट ड्रिंक्स से परहेज करें क्योंकि ये आपको डिहाइड्रेट कर सकते हैं|

3. संतुलित आहार: संतुलित आहार लें और इसमें साबुत रोटी, सब्जियां, सलाद, अनाज, ताजे फल और सूखे मेवे शामिल करें| अपने आहार में दही या प्रोबायोटिक्स शामिल करें| भोजन को ना छोड़े - क्योंकि भोजन आंतों की गतिशीलता को उत्तेजित करता है|



4. नियमित शौच की आदतें विकसित करें: कभी भी शौच जाने की इच्छा की उपेक्षा न करें| नियमित रूप से शौचालय जाने की आदतें विकसित करें-नियमित समय, सही पोजीशन और सही पेट का दबाव|

5. कब्ज पैदा करने वाली दवाओं से बचें (उदाहरण के लिए एनाल्जेसिक, एंटासिड्स, एंटीकॉलाइनर्जिक एजेंट्स, एंटीस्पेज्मोडिक, एंटीडीयरेहेल्स, एनटिपीलेप्टिक,  एंटीहिस्टामाइनस, आईरन सप्लीमेंट्स, खांसी की दवाइयां, सीसीबी, कोडीन आदि)

5 Tips to prevent constipation

Constipation is the difficult passage of small hard stools. Here are the 5 tips to prevent constipation.

1. Adequate hydration: Take plenty of fluids (at least 2.5 to 3 liters per day) especially water and fruit juices (for example prune juice). Avoid coffee and soft drinks as it can dehydrate you.

2. Balanced diet: Take a balanced diet and include wholemeal bread, vegetables, salads, cereals, fresh, and dried fruits that provide bulk. Include curd/ yoghurt/ probiotics in your diet. Don't miss meals-food stimulates gut motility.

3. Mobility: Avoid a sedentary lifestyle and do adequate exercise daily- especially walking (for at least 30 minutes to 1 hour) is important.

4. Develop regular toilet habits: Never neglect the call to stool/ toilet. Develop regular toilet habits- regular timing, proper positioning, and abdominal pressure.

5. Avoid medications and compounds causing constipation (for example analgesics, antacids, anticholinergics agents, antispasmodics, antidiarrheals, antiepileptic, antihistamines,  iron supplements, cough mixtures, CCBs, codeine, etc.).

विश्व एड्स दिवस 2020: "वैश्विक एकजुटता, साझा जिम्मेदारी" समय की माँग

इसकी जरूरत क्या है?

1990 से दुनिया ने काफी प्रगति की है, लेकिन फिर भी एचआईवी एक प्रमुख वैश्विक सार्वजनिक स्वास्थ्य मुद्दा है| कोविड-19 महामारी के दौरान, एचआईवी भी कई अन्य स्वास्थ्य मुद्दों की तरह अतिरिक्त चुनौतियों का सामना कर रहा है| 2019 में करीब 38,000,000 लोग एचआईवी के साथ जी रहे थे और 1,700,000 नए लोग एचआईवी से संक्रमित हुए थे| 2019 में करीब 320,000 नए बच्चे और किशोर एचआईवी से संक्रमित हुए थे, यानी प्रत्येक 100 सेकेंड्स में एक संक्रमण| 690,000 लोगों की एचआईवी से जुड़े कारणों से मृत्यु हो गई और 68% लोग (वयस्क) एंटीरेट्रोवायरल थेरेपी (A R T ) लेते हुए एचआईवी के साथ जी रहे थे|आमतौर पर एचआईवी लोगों को सबसे अधिक उत्पादकता के उम्र में प्रभावित करता है जिससे समय से पहले मृत्यु हो जाती है और इससे पूरे परिवारों, समुदायों और देशों की सामाजिक, आर्थिक संरचना बुरी तरह प्रभावित होती है| इसलिए हमें इस बीमारी (एचआईवी/ एड्स) के बारे में लोगों को शिक्षित करने और जागरूकता बढ़ाने की बहुत आवश्यकता है| 


एचआईवी क्या है?

एचआईवी- मतलब हुयूमन इम्यूनोडिफिशिएंसी वायरस| मानव शरीर में प्रवेश करने के बाद एचआईवी धीरे धीरे प्रतिरक्षा प्रणाली अर्थात संक्रमण अथवा बीमारियों से लड़ने की शारीरिक क्षमता को नष्ट कर देता है|

एड्स क्या है?

एड्स- मतलब एक्वायर्ड इम्यूनोडिफिशिएंसी सिंड्रोम| यह एचआईवी इनफेक्शन/संक्रमण के बाद की स्थिति है| यह एक ऐसी स्थिति है जिसमें प्रतिरक्षा प्रणाली बहुत कमजोर हो जाती है और लक्षणों का एक समूह प्रकट होता है| एचआईवी संक्रमण के समय से लेकर एड्स की स्थिति तक लगभग 8 से 10 साल लगते हैं| ए आर टी के उपयोग से एचआईवी संक्रमित व्यक्ति भी वर्षों तक लक्षण मुक्त एवं उत्पादक जीवन जी सकता है|

यह कब मनाया जाता है?

विश्व एड्स दिवस 1988 के बाद हर साल 1 दिसंबर को मनाया जाता है| यह वैश्विक सार्वजनिक स्वास्थ्य समस्या के रूप में आम लोगों में जागरूकता बढ़ाने, उन्हें शिक्षित करने और एचआईवी के प्रति समझ में सुधार लाने के लिए समर्पित है| यह एड्स को समाप्त करने में प्रगति और सार्वभौमिक स्वास्थ्य कवरेज तथा स्वास्थ्य के अधिकार की दिशा में प्रगति के बीच की निर्भरता को समझने का एक बहुत अच्छा अवसर प्रदान करता है| इस वर्ष विश्व एड्स दिवस की थीम "वैश्विक एकजुटता, साझा जिम्मेदारी" है|

इसके चिन्ह और लक्षण क्या है?

यह संक्रमण के स्टेज पर निर्भर करता है| 

  • प्रारंभिक संक्रमण के बाद पहले के कुछ हफ्तों में, लोगों को कोई लक्षण नहीं होता या इन्फ्लूएंजा जैसी बीमारी का अनुभव हो सकता है जैसे कि बुखार, सिर दर्द, फुंसियां, गले में  खराश आदि| 
  • जैसे-जैसे संक्रमण बढ़ता है, व्यक्ति अन्य चिन्ह और लक्षण विकसित कर सकता है जैसे कि लिंफ नोड्स में सूजन, वजन का घटना, बुखार, दस्त और खांसी|
  • अगर उपचार ना किया जाए तो, वे अनेक गंभीर बीमारियों जैसे कि टीवी या तपेदिक, मेनिनजाइटिस, गंभीर जीवाणुओं का संक्रमण और कैंसर के शिकार हो सकते हैं|

क्या एचआईवी संक्रमित हर व्यक्ति को एड्स होता है?

नहीं, एचआईवी का डायग्नोसिस होने का यह मतलब बिल्कुल नहीं है कि व्यक्ति को एड्स का भी डायग्नोसिस किया जाएगा| चिकित्सकों द्वारा एड्स तभी डायग्नोस किया जाता है, जब एचआईवी से संक्रमित व्यक्ति अनेक प्रकार के गंभीर अवसरवादी संक्रमण से संक्रमित हो जाता है और उनका सीडी 4 सेल काउंट एक निश्चित स्तर से नीचे गिर जाता है|

लोग एचआईवी से कैसे संक्रमित होते हैं?

  • एचआईवी संक्रमित व्यक्ति के साथ और सुरक्षित गुर्दा योन सेक्स- एचआईवी का संक्रमण सबसे अधिक एक संक्रमित व्यक्ति के साथ असुरक्षित यौन संबंध बनाने से होता है| 
  • एचआईवी संक्रमित रक्त या रक्त उत्पादों का ट्रांसफ्यूजन/ आधान- संक्रमित रक्त का आधान, रक्त उत्पाद या अंग अथवा उत्तक प्रत्यारोपण जो एचआईवी से संक्रमित होते हैं, प्राप्त करने से भी एचआईवी का संक्रमण हो सकता है| 
  • एचआईवी संक्रमित रक्त से दूषित सुयोग को साझा करने से- एचआईवी, संक्रमित रक्त से दूषित सूई और सिरिंज के माध्यम से भी प्रसारित हो सकता है|
  • एचआईवी संक्रमित मां से उसके बच्चे में-गर्भावस्था के दौरान, जन्म के दौरान या स्तनपान के माध्यम से प्रसव के बाद भी एचआईवी संचारित हो सकता है|
  • आकस्मिक नीडल स्टिक इंजरी का अनुभव (जैसा कि स्वास्थ्य कर्मियों के बीच) और अन्य यौन संचारित संक्रमण (STI) जैसे कि हरपीस, chlamydia, गोनोरिया तथा बैक्टीरियल वेजाइनोसिस संक्रमण के अन्य रिस्क फैक्टर है|

हाई रिस्क ग्रुप्स (HRG) कौन हैं?

  • महिला यौन कर्मी(FSW)
  • पुरुषों के साथ यौन संबंध रखने वाले पुरुष(MSM)
  • ट्रांस जेंडर्स (TG) या हिजड़े
  • नशीली दवाइयों का उपयोग करने वाले लोग उच्च जोखिम समूह में आते हैं|

पूल या ब्रिज आबादी कौन है?

वैसे लोग जिनके उच्च जोखिम वाले समूहों में यौन साथी होते हैं साथ ही साथ कम जोखिम जैसे कि सामान्य आबादी मैं भी यौन साथी होते हैं उन्हें पूल या ब्रिज आबादी कहा जाता है क्योंकि वे लोग उच्च जोखिम समूह से सामान्य आबादी तक एक संचरण का पुल बनाते हैं| उच्च जोखिम समूहों के साथ निकटता के कारण ट्रक ड्राइवरों और प्रवासी श्रमिकों को पुल या ब्रिज आबादी भी कहा जाता है और एचआईवी से संक्रमित होने के लिए बहुत अधिक रिस्क पर होते हैं|

एचआईवी कैसे नहीं फैलता है?

एचआईवी सामान्य सामाजिक संपर्कों से नहीं फैलता है जैसे कि 

  • गले मिलना 
  • हाथ मिलाना 
  • शौचालय साझा करना 
  • साथ में खाना खाना 
  • सामाजिक चुंबन
  • मच्छरों टिक्स या अन्य रक्त चूसने वाले कीड़े
  • एक ही बस कार या ट्रेन में यात्रा करना

क्या एचआईवी या एड्स के लिए कोई उपचार उपलब्ध है?

पूरी तरह से ठीक करने का कोई इलाज उपलब्ध नहीं है, लेकिन प्रभावी एंटीरेट्रोवायरल ट्रीटमेंट (ART )दवाएं उपलब्ध है जो एचआईवी पॉजिटिव व्यक्ति के जीवन को लंबा कर सकती हैं और जीवन की गुणवत्ता को बढ़ा सकती हैं| ए आर टी दवाइयों को एक बार शुरू करने के बाद रोगी को जीवन भर लेना पड़ता है| यह भारत के ए आर टी केंद्रों पर मुफ्त में उपलब्ध है|

एचआईवी / एड्स का रोकथाम और नियंत्रण कैसे किया जाता है?

  • अपने यौन साथी के प्रति वफादार रहें और कंडोम के उपयोग के साथ सुरक्षित यौन व्यवहार करें|
  • जब भी जरूरत हो यौन रोगों के लिए परीक्षण और उपचार करवाएं|
  • सुई, सिरिंज और स्वाब सहित सुई या अन्य इंजेक्शन लगाने वाले उपकरण कभी भी साझा ना करें|
  • सभी गर्भवती महिलाओं का रूटीन एंटीनेटल स्क्रीनिंग के एक भाग के रूप में एचआईवी का परीक्षण किया जाना चाहिए और पॉजिटिव पाए जाने पर तुरंत उपचार शुरू किया जाना चाहिए|
  • पोस्ट एक्सपोजर प्रोफाइलेक्सेस (PEP): यह एचआईवी संक्रमण की संभावना को कम करने के लिए एक अल्पकालिक एंटी रेट्रोवायरल उपचार है, जो व्यवसायिक रूप से या संभोग के माध्यम से संक्रमण के जोखिम के बाद दिया जाता है|

मूल संदेश-

  • जागरूकता से ही एड्स की सबसे अच्छी रोकथाम संभव है|
  • एचआईवी संक्रमण का कोई इलाज नहीं है, हालांकि प्रभावी एंटीरेट्रोवायरल दवाएं वायरस को नियंत्रित करती हैं और संचरण को रोकने में मदद करती हैं|
  • एंटीरेट्रोवायरल थेरेपी की मदद से एचआईवी वाले लोग और उच्च जोखिम वाले लोग स्वस्थ, लंबे और उत्पादक जीवन का आनंद ले सकते हैं|

कोविड-19 के क्या प्रभाव हुए हैं?

कोविड-19 के कारण आवश्यक एचआईवी सेवाओं में विघटन हुआ है, जिससे कि नाजुक स्वास्थ्य प्रणालियों वाले देशों में एचआईवी की रोकथाम परीक्षण, उपचार और देखभाल सेवाएं बाधित हो रही हैं| इससे वैसे लोग जिन्हें एचआईवी एड्स का खतरा ज्यादा है, उनके जान को अधिक खतरा उत्पन्न हो गया है| इस वर्ष 1 दिसंबर को विश्व स्वास्थ्य संगठन और पूरी दुनिया, कोविड-19 और उसके दौरान एचआईवी सेवाओं को बनाए रखने के लिए  उन सभी लोगों का धन्यवाद कर रहे हैं जिन्होंने वैश्विक एकजुटता दिखाते हुए एचआईवी को कम करने के लिए हाथ मिलाया है| यह कमजोर समूह पर ध्यान केंद्रित करने का समय है जो पहले से ही ज्यादा जोखिम में है और बच्चों तथा किशोरों के लिए सेवाओं का विस्तार करने की जरूरत है|

What is the need?

Since the late 1990s, the world has made significant progress but HIV remains a major global public health issue. It is facing additional challenges during the COVID-19 pandemic like many other major health issues. In 2019 around 38,000,000 people were living with HIV, 1,700,000 people were newly infected with HIV, 320,000 children and adolescents newly infected with HIV in 2019, 1 every 100 seconds and 690,000 people died of HIV-related causes & 68% of adults living with HIV received lifelong antiretroviral therapy (ART). HIV generally affects people at the most productive age, leading to premature death thereby severely affecting the socio-economic structure of whole families, communities, and countries. Hence we need to educate people about this disease & raise awareness.

What is HIV?

HIV - Human Immuno-deficiency Virus. After entering the human body HIV gradually destroys the immune system, i.e. the ability of the body to fight infections/diseases.

What is AIDS?

AIDS - Acquired Immune Deficiency Syndrome. It is the later stage of HIV infection. It is a condition in which the immune system becomes very weak & a group of symptoms appears. It takes around 8-10 years from the time of HIV infection to the stage of AIDS. HIV infected person can also lead to symptom–free and productive lives for years.

When is it celebrated?

World AIDS Day is celebrated every year on December 1 since 1988. It is dedicated to raise awareness, educate and improve the understanding of HIV to common people as a global public health problem. It provides a very good opportunity to understand the interdependence between progress in ending Acquired Immunodeficiency Syndrome (AIDS) and progress towards universal health coverage and the right to health. this year the theme of World AIDS Day is “Global solidarity, shared responsibility”.

What are the Signs and Symptoms?

  • It varies depending on the stage of infection. The first few weeks after initial infection, individuals may experience no symptoms or an influenza-like illness including fever, headache, rash, or sore throat.
  • As the infection progresses, an individual can develop other signs and symptoms, such as swollen lymph nodes, weight loss, fever, diarrhea, and cough.
  • Without treatment, they could develop severe illnesses such as tuberculosis, meningitis, severe bacterial infections, and cancers.

Do all people with HIV have AIDS?

No. Being diagnosed with HIV does not mean that a person will also be diagnosed with AIDS. Doctors diagnose AIDS only when people with HIV infection begin to get severe opportunistic infections (OIs), or their CD4cell counts fall below a certain level.

How do people get infected with HIV?

HIV can be transmitted through: 

  • Unprotected anal or vaginal sex with an HIV infected personThe most frequent mode of transmission of HIV is through sexual contact with an infected person.
  • Transfusion of HIV-infected blood or blood productsReceiving blood transfusions, blood products, or organ/tissue transplants that are contaminated with HIV can result in HIV infection.
  • Sharing of needles contaminated with HIV infected bloodHIV can be transmitted through needles and syringes contaminated with HIV infected blood.  
  • From HIV infected mother to her baby – during pregnancy, during birth, or after delivery through breast milk.
  • Experiencing accidental needle stick injuries, including among health workers and having another sexually transmitted infection (STI) such as syphilis, herpes, chlamydia, gonorrhea, and bacterial vaginosis are other risk factors.

Who are High-Risk Groups (HRGs)?

  • Female sex workers (FSW)
  • Men who have sex with men (MSM)
  • Transgenders (TG)/Hijras' and 
  • Injecting drug users (IDUs) belong to high-risk groups 

Who are Bridge Populations?

Individuals who have sexual partners in the high-risk groups, as well as other partners of lower risk (general population), are called a “bridge population” because they form a transmission bridge from the HRGs to the general population. Truckers and Migrant workers are named as bridge populations due to their close proximity with high-risk groups and are at the risk of contracting HIV.

How HIV is not transmitted?

HIV doesn’t spread through ordinary social contact; for example by

  • Hugging
  • Shaking hands 
  • Sharing toilets
  • Sharing dishes 
  • Closed-mouth or “social” kissing with someone who is HIV-positive
  • Mosquitoes, ticks, or other blood-sucking insects
  • Traveling on the same bus/ car/ train

Is there any treatment available for HIV/AIDS?

There is no cure but effective Anti-Retroviral Treatment (ART) drugs are available which can prolong the life of an HIV-positive person and thus enhance the quality of life. The patient has to take life long treatment once initiated on ART. It is freely available at ART centers across India.

How to prevent and control HIV/AIDS?

  • Be faithful to your partner & Practice  safe sexual behaviors such as the use of condoms
  • Get tested and treated for sexually transmitted diseases
  • Never share needles or other injecting equipment, including syringes, spoons, and swabs (injecting drug users)
  • All pregnant women should be tested for HIV as a part of routine antenatal screening, and start HIV treatment immediately if found positive for HIV.
  • Post-exposure prophylaxis (PEP): It is a short-term antiretroviral treatment to reduce the likelihood of HIV infection after potential exposure, either occupationally or through sexual intercourse.

Key messages-

  • Awareness is the best prevention of AIDS.
  • There is no cure for HIV infection. However, effective antiretroviral (ARV) drugs can control the virus and help prevent transmission.
  • With antiretroviral (ARV) therapy, people with HIV, and those at substantial risk, can enjoy healthy, long, and productive lives.

What are the impacts of COVID-19?

HIV prevention, testing, treatment, and care services are being disrupted mostly in countries with fragile health systems due to a breakdown in essential HIV services due to COVID-19. It is threatening lives & will leave many vulnerable populations at greater risk of HIV infection and AIDS-related deaths. On 1 December WHO & the whole world are paying tribute to all those working to provide HIV services & joining hands for “global solidarity” to maintain essential HIV services during COVID 19 and beyond. It is a time to focus on vulnerable groups who are already at risk and expand coverage to children and adolescents.
